MOSCOW — Separatist forces in the eastern Ukrainian stronghold of Slovyansk brought down two government helicopters on Friday, killing two of their occupants, the Ukrainian Defense Ministry said, as government forces launched what was termed the “active phase” of an assault to dislodge militants.
